Wellness

The initiative began in 2008 consisting of seven community support agencies providing services throughout their respective counties. Wellness for Seniors (WFS) of Perth and Huron has been funded through the South West Local Health Integration Network (LHIN) to create a wellness program based on the enhancement of existing programs and the creation of new ones. The WFS programs bring wellness, education and health monitoring to community dwelling individuals in rural and isolated areas. Promoting and sustaining improved health for older adults through a standardized best practice model of programs in local communities across Huron-Perth. All services will be provided with the overarching themes of education, exercise and health promotion/monitoring. WFS programs encourage individuals to manage their own health. Wellness programs have been created to provide a safe, inexpensive avenue for individuals to remain active and healthy.

The use of volunteers for program delivery allows for a high volume of programming with low costs of delivery. All seven agencies partnering in this project have collaborated in program delivery and development, along with community partners including Family Health teams, Family Physicians, Health Units, Community Professionals and other agencies.

Health Education

Education Sessions

Coffee Hours

  • Coffee, tea, juice and snacks are provided
  • Free of charge

Bathing

Theraputic baths can be scheduled weekly or as needed at the West Perth Village in the hyrdosound tubs. Baths are done by trained healthcare staff.

Rates: $22.00/half hour
(a $22.00 cancellation fee is charged if baths are not cancelled the day before)

The Hydrosound tub is a low frequency ultrasound which propagates pressure waves in water. These waves continuously create millions of microscopic bubbles and remove dirt particles from the client’s skin, gently and effectively. The Hydrosound’s gental massaging action offers a new standard of cleanliness without discomfort. The benefits of the bathing program hydrosonic tub are:

  • personal hygiene
  • promotes healing of wounds
  • improves circulation
  • a safe alternative to bathing for those who need assistance

Mitchell & Area Community Outreach provides a wide range of community and home support services to seniors, the disabled and those recuperating from injury, illness and accident in the Municipality of West Perth so that they are able to remain living independently in their homes. This support also helps family and friends in their role as caregiver, and fosters strong, supportive communities. Services are provided by professional staff and dedicated volunteers who recognize one’s wish to remain in their chosen place of residence as long as possible.

West Perth Transit

Mitchell & Area Mobility Bus is a door-to-door transportation service for seniors, the disabled and those recuperating from injury, illness and accident in the Municipality of West Perth. This service has been in operation since 1985 and now has 2 mobility vehicles to meet client needs. One vehicle is up to 16 passengers bus and the other is an accessible mini-van

Preferred Accommodation

Private Room – Daily Rate for our new Home will be 63.73 – and Monthly will be $1,938.46

Daily Service Rates

  • $4.50/One way trip in town

  • (Addtional stops in between are $4.00/stop)

  • $10.00/one way trip in township (West Perth)

Rates

  • For travel within Perth & Huron Counties – $0.85/km

  • For travel outside Perth & Huron Counties – $0.70/km

  • Waiting fee of $18.00/hr will be charge once the client is dropped off until pickup time

  • If the trip is one way only for the client, the return kilometer rate will be charged

Charter Rates

  • $48.00/hr for driving

  • $38.00/hr for waiting/loading/standby

  • There is a one hour minimum charge of $38.00 for all charters booked for less than one hour

  • For West Perth residents traveling within West Perth – $38.00/hr for both driving and waiting/loading/standby

Bus Pass

  • In town: $92.00 for 25 trips
  • Township: $184 for 25 trips

Booking Information

To book transportation, call Easyride at 519-272-9875 from 8:30am – 4pm. 24hours notice for booking is required but last minute appointments may be accommodated.

Cancellation policy – Cancellations should be made as soon as possible to accommodate others. A one way rate charge will apply to cancellations at time of pickup or if a client is not home at the requested time of pickup.

Transportation Assistant

Trained support workers assist clients to medical appointments. Assistance could include, but is not limited to: help with dressing, companionship, feeding, getting something to eat, etc.

The support worker meets the client at their place of residence and is with them from the time they arrive, during the appointment and accompanies the client home.

About The Bus

Mitchell & Area Mobility Bus History

Mitchell & Area Mobility Bus began transporting clients in West Perth in 1985. It started as a result of a seniors’ needs study conducted in 1984 which indicated a need for specialized transportation services. The service was officially launched on October 1, 1985 with one service day.

Delivery Dates

Meals are delivered Mondays, Wednesdays and Fridays between 11:45am and 12:30pm. Meals are not delivered on statutory holidays. Please check the Ordering Meals page to see where we deliver and on what days.

Hot Meals

Nutritious meals are delivered by volunteers to individuals who need assistance with their nutritional needs. This may include people who need meal preparation assistance, have special dietary needs identified by a health professional or just wish to have a balanced meal delivered to their home. Deliveries can be arranged for one or multiple days with the option of multiple meals each day. Special dietary needs will be accommodated when possible.

Frozen Meals

Apetito frozen meals are also available to supplement hot meals or as an alternative. These are great to store in the freezer for future use. Check out the Apetito Website for more information on their products.

Ordering Meals

Meal Requests
To request a meal you must call the Outreach Office at 519-348-9765, at a minimum, before 9:30 am on the date of the delivery. You are not able to make same day requests after this time.

Meal Cancellations
To request a meal you must call the Outreach Office before 9:30 am on the date of the delivery. Any meals that are not cancelled by this time will be charged, even if it is returned. If you decide you no longer need the service and cancel before 9:30am there is no charge and you will be reimbursed in full for any meals that you had prepaid.

Meal Costs
Meal costs change on a regular basis due to fluctuations in the cost of preparing them. For more information on the costs please Contact Us.

Outreach website under Swimming/Aquafit to “45 minute class for 10 weeks offered at a neighbouring community pool” and the cost is approximately $170/person

Also can you remove the Tuesdays at the Dublin church and change it to 9:30 @ Upper Thames Missionary Church and change the Thursdays @ 9:30 to Thursdays @ 6:30 @ West Perth Community Centre.

Under Health Assessments can you change CHAP to Blood Pressure Clinics (we no longer use the term CHAP):

– 2nd Tuesday/month @ 1-3pm at West Perth Public Library (drop-in)

– Last Tuesday/month @ 1-3pm at the Ritz Manor Apartments Lounge (by-appointment)

– Program to help monitor blood pressure for those who are at risk or are diagnosed with high blood pressure (hypertension)

– Free of charge

– Cost is now $35.00 for everyone (no discount for returning clients)

Swimming/AquaFit

  • 45 minute class for 10 weeks offered at Spruce Lodge Therapeutic Pool in Stratford
  • Travel by Mobility Bus to destination
  • Approximately $210/person (includes swimming admission, transportation and HST)

Exercise & Falls Prevention Classes
Group Fitness Class – 60 minutes Group exercise (level 3)
Level three classes are offered in community settings. The class is designed for those with a moderate to vigorous level of ability, providing the option to participate at a higher level of intensity. Class is broken down into 6 sections: Warm-up, cardio, cool-down, balance, strength & stretching. Classes open to the public.

Group Fitness Gentle Class – 40 minute Active Wellness (Level 2)
Level two classes are offered in community and retirement settings. The class is designed for those with a moderate level of ability. This level of class is a mix of both standing and seated components. This gentler version of our Group Fitness class is great for individuals with limited mobility or recovering from injury. Classes open to the public.

Classes are provided at no cost with funding from the South West Local Health Integration Network (LHIN).
Classes are enhanced with falls prevention education modules discussed throughout class.

Home Support Exercise Program

  • 10 simple & progressive exercises for frail or fragile individuals offered in-home
  • Program is 10 weeks long
  • Trained individual comes into home and helps the client perform exercises once/week. The client is asked to perform exercise 2-3 times/week on their own.
  • Dates and times are catered to individual clients
  • $2.00/session ($20.00/10 weeks)

Nintendo Wii

  • Fridays @ 2:00-3:00pm
  • Enjoy socializing and Wii games
  • Sessions held in the Ritz Manor Apartments Lounge
  • Free of charge

Health Assessments

Audiology Clinic

  • Hearing assessment & hearing aid cleanings done by an Audiologist
  • Service provided by Connect Hearing Stratford
  • Session provided every 6-12 months

CHAP Program (Cardiovascular Health Assessment Program)

  • 2nd Thursday/month @ 9:00am – 12:00pm at Mitchell Friendship Centre
  • 3rd Monday/month @ 1:00pm – 3:00pm at Dublin St. Patrick’s Church (bumped to 2nd Monday if 3rd falls on a holiday)
  • Last Tuesday/month @ 1:00pm – 4:00pm at the Ritz Manor Apartments Lounge
  • Drop-in program to help monitor blood pressure for those who are at risk or are diagnosed with high blood pressure (hypertension)

SCREEN Program (Seniors in the Community Risk Evaluation for Eating & Nutrition)
Seniors in the Community Risk Evaluation for Eating & Nutrition (SCREEN) was developed to determine if a senior is at nutritional risk through a questionnaire. Clients are then provided with healthy living and nutritional information. The questionnaire can be filled out over the phone or can be done in the home.

Foot Care Clinic

  • Monthly (contact Outreach for specific dates, appointment required)
  • Registered Foot Care nurse provides foot & toenail treatments
  • Cost is $30.00/initial appointment/consultation, $25.00/returning clients

Senior Fitness Test

  • Held semi-annually during scheduled exercise classes
  • Assesses individual’s strength, cardiovascular endurance, flexibility, balance and ability
  • Free of charge

Social & Congregate Dining

Social & Congregate Dining programs promote health and wellness through a variety of activities.

To register for one of our various programs please Contact Us today. For most events, at least one week’s notice, prior to the event date, is required to guarantee you a spot. To note, some events have deadlines response dates. Please take a look at our Calendars for upcoming events. New clients will have to answer a few questions prior to being able to attend any event for health and safety purposes.

Neighbourly Services

Neighbourly Services programs designed to help individuals remain living independently in their home. These programs include:

Volunteer Transportation

Volunteers provide transportation to clients for out of town medical appointment. Outreach staff will match clients with volunteers who will provide the trip.

Clients pay a fee to the drivers which reimburses them for their fuel. Reimbursement is based on a per kilometre fee. Outreach Office staff will inform the client about the full cost of the trip when the staff call back to confirm that a driver has been booked. Clients must pay the driver directly at the time of pickup for the trip. If there are any parking fees incurred, the client will also pay for them at the parking area.

Clients are required to inform staff of any extra stops that need to be made when booking their trip, so that the cost can accurately be calculated. Unless it is a medical necessity (i.e. stop at the pharmacy), the driver will not make any extra stops that have not booked ahead of time. If there needs to be a stop for medical reasons that has not been booked prior to the trip, it will be at the discretion of the driver if they will be able to accommodate the stop. The client will have to pay $3.00 at that time for that stop. As well, if the client lives outside of Mitchell, there will be an extra $3.00 for picking fee in the rural area.

At no time is the client to call the volunteer at their home. If a booking needs to be changed or cancelled or a trip added, the client must call Easyride.

If the volunteer arrives for trip pickup at the client home, and the client is not home or denies the trip at that time, the client will be billed for the trip.

Clients must book a minimum of 24 hours in advance of their trip.

Friendly Visiting

Friendly Visitors are volunteers who are interested in visiting with clients who live in their own homes and would like some extra socialization. Volunteers provide companionship and support to help individuals retain their independence.

A Friendly visitor can visit in a client’s home, play games, go for a walk, visit at the local coffee shop or activities of mutual interest to the volunteer and client.

There is no fee for this service as it is provided by a volunteer

Volunteers are matched with clients based on factors such as location, similar interests and personalities. We try very hard to make sure volunteers and clients will be compatible. Careful screening is used when matching volunteers with clients. All volunteers go through a police screening process as well as reference checks and interviews.

Grocery Assistance

Groceries are ordered by the client by calling Walkom’s Valumart (519-348-8446) and requesting items. Volunteers pickup and deliver groceries to the client’s home. Groceries and delivery fee is paid directly to Valumart when ordering groceries.

Orders must be placed by 3:30PM on the Monday to receive groceries on the following Tuesday.

Home Help/Home At Last

Mitchell & Area Community Outreach works in partnership with ONE CARE Home & Community Support Services to provide Home Help/Home Support and Home At Last Services. Call our office at 519-348-9765 for more details.

Home Help/Home Support
This program provides assistance with household chores and daily tasks such housekeeping, laundry, in-home meal preparation to name a few. Respite care can also be provided when the family care-giver needs a break. Work done is individual to the person’s needs.

The cost varies depending on the length of time utilized

Home at Last (HAL)
Home At Last is a free service to help aging adults and those with special needs to settle in at

Settle in at home safely and comfortable after a hospital stay.

How Does Home at Last Work? HAL identifies seniors in local hospitals in Huron and Perth Counties who have limited supports upon discharge or potential concerns regarding how well they may settle when they return home. Upon discharge from hospital, transportation services are available to get the client safely home and a Personal Support Worker (PSW) will stay with the client for up to four hours. Once home, the PSW will do a safety check of the home, and provide personal care and homemaking services. A complimentary meal is provided, and if needed, the PSW can pick up some groceries and/or prescriptions for the client. Time is given to answer questions and to review support agencies available in the community to meet the client’s particular needs. Clients also receive a follow–up call within 48 hours to ensure the client is managing well.

Referrals to Outreach programs can be made in a variety of ways.

The most common ways to start service are to contact the Outreach Office either by visiting the office or by calling 519-348-9765. Other methods include fax or e-mail. Please see our Contact Us page for more details to get ahold of us.

An individual can call or visit the office to start service. A referral from a doctor or health professional is not required. However, other individuals such as family members, neighbours, friends and/or health professionals can also call on your behalf to set-up/start service.

To become a volunteer there are a few items that need to be completed. Interested individuals are required to complete an application form, provide 2 references and complete a criminal record check. A confidentiality form will also have to be signed. The confidentiality form ensures that all volunteers are aware that all matters pertaining to the operations of Mitchell & Area Community Outreach or personal information pertaining to the clients, members or families involved with Outreach is kept private and confidential. There will also be an interview with the Volunteer Coordinator.
